should properly maintain their breathing system (at least yearly maintenance).  Perform pre-dive check of equipment.  Monitor your pressure guage!  PRACTICE OUT OF AIR PROCEDURES -- you have three to five minutes during your safety stop....why not use this time to practice out of air techniques.
Weight System Failure.  Far too many bodies are recovered with their weight belt still attached.  Too much weight and inability to remove weight continues to be a leading cause in diving injuries.  STRIVE FOR PROPER WEIGHTING -- if you are not sure, ask someone to help you.  KNOW HOW TO REMOVE YOUR WEIGHTS AND CHECK YOUR EJECTION SYSTEM PRE-DIVE.

Decompression Illness and Rate of Ascent. 

EVERY DIVE IS A "DECOMPRESSION DIVE". 
A "safety stop" is a decompression stop. The 60 feet per minute ascent rate was decided upon based on a compromise between the US Navy (they wanted 100 feet per minute) and some commercial divers (they
